Home Health Links is building PTA coverage in Downtown LA/ Silver Lake. This position is intended for therapists who want flexible home health visit volume, local routes, and one-on-one care with older adults in the home setting.
Coverage area
What you will do
- Complete appropriate home health visits
- Communicate schedule and patient updates
- Complete documentation within expected timelines
- Coordinate with HHL support when questions or scheduling issues come up.
What we are looking for
- Active California license for the listed discipline
- Reliable transportation
- Ability to document on your own device
- Comfort communicating with patients, families, agencies, and the HHL team.
